This warranty statement was included with every vehicle from the factory. Many owners would put it in the glove box with the owners manual etc. 

Second picture shows originals that were sent to a dealer in the mail.

Third picture shows the paper statement with the paperwork Ed Meyer helped me put together for a 1970 BOSS 429 we restored years ago. The picture shows the statement paper upside down within the plastic trash bag that the Ford dealers would put in your glove box. I really don't know why I didn't take it out of the bag when I took the picture. ~Jack
